Application for New Connection

How to apply Electric Service Connection:
1. Attendance to Pre-Membership orientation Seminar (PMOS) scheduled every Thursday 8:00AM-11:00AM at SURNECO Main Office;  
2. Get an Application for Electric Service Connection Form from Consumer Welfare Desk (CWD) at Main Office/ Sub-Area Offices;
3. Fill-up application form and comply all requirements needed (pls. refer requirements below)
4. If requirements already completed, submit the application form to CWD for evaluation, and then, actual house wiring inspection will follow (only if house wiring installation already finished);
5. After inspection, inquire to CWD the necessary fees and pay its corresponding amount to teller in charge;
6. Secure Billing Clearance from Main Office (CSD Department) to be attached in the application form, then submit to Internal Audit (IA) for verification and to CCPRO Department for membership application;
7. Submit application form to CWD for Turn-On Order and schedule of meter execution;
8. If necessary, follow-up electric service connection (turn-on) to Main office for applicants from Surigao City and to Sub-Area offices for applicants from respective municipalities.
1. Special Power of Attorney (SPA) if Representative w/ Photocopy of valid ID
2. Photocopy of Valid ID (Driver's License, SSS, GSIS, etc.)
3. Proof of Ownership (Lot Title, Tax Declaration of Land & Tax Declaration of Building, Deed of Sale, etc. - Notarized authorization letter from lot owner/administrator w/ photocopy of valid ID)
4. Barangay Clearance (Location address of Electric Service Connection)
5. Barangay Certificate if land is under Brgy. Site or Public Land
6. Electrical Plan - Approved and Sealed by P.E.E.
7. Electrical Permit - secured from the City Engr.'s or Municipal Engr.'s Office
8. Certificate of Attendance  - Fire Safety Lecture from BFP
9. Secure Membership Application Form at CCPRO - Attend the Pre-Membership Orientation Seminar (PMOS) every Thursday 8:00am - 11:30am.
10. Photocopy of Birth Certificate (if single), Marriage Contract (If Married) SURNECO FORMS: (Available after attending the PMOS)
11. Billing Clearance from Consumer Services Dept. & Internal Audit
12. Application Form will be printed if all the requirements were approved
13. Member-Consumer's Commitment & Obligation-Notarized
14. Affidavit of Waiver - Notarized (if Gov't and Privately owned)
